Each volunteer LifeLine Pilot...
Will wear their LifeLine Pilots' name badge and cap so that they are easily identified by flight recipients and passengers. As a LifeLine Pilot, you will receive your first cap free of charge. Additional caps may be purchased and charged to your credit card by calling the office, 309-697-6282;
Will communicate with the Flight Coordinator and report any flight or schedule change as soon as possible;
Is responsible for completing and sending the Day of Mission: Flight Authorization, Liability Release and Indemnity form and Mission Follow-Up form to our office following the completion of the flight;
Will provide a report describing the circumstances of any aviation accident or incident, regardless of whether on a mission or a personal flight, to LifeLine Pilots. This will be reviewed by the Safety Committee;
Agrees that they will not charge a fee, accept an honorarium or donation from any passenger or other party wishing to reimburse the Pilot for expenses incurred as a result of any LifeLine Pilots' flight;
Wishing to remain an active pilot, will respond to the annual Renewal Letter and provide updates for the database.
