Each volunteer LifeLine Pilot...

Will wear their LifeLine Pilots' name badge and cap so that they are easily identified by flight recipients and passengers. As a LifeLine Pilot, you will receive your first cap free of charge. Additional caps may be purchased and charged to your credit card by calling the office, 309-697-6282;

Will communicate with the Flight Coordinator and report any flight or schedule change as soon as possible;

Is responsible for completing and sending the Day of Mission: Flight Authorization, Liability Release and Indemnity form and Mission Follow-Up form to our office following the completion of the flight;

Will provide a report describing the circumstances of any aviation accident or incident, regardless of whether on a mission or a personal flight, to LifeLine Pilots. This will be reviewed by the Safety Committee;

Agrees that they will not charge a fee, accept an honorarium or donation from any passenger or other party wishing to reimburse the Pilot for expenses incurred as a result of any LifeLine Pilots' flight;

Wishing to remain an active pilot, will respond to the annual Renewal Letter and provide updates for the database.

• LifeLine Inc. dba LifeLine Pilots is a non-profit, 501 (c)(3) organization based in Peoria, IL.
• Missions are broadcast by e-mail; Pilots need to check their e-mail at least every other day; If you do not have e-mail or if we are having difficulty filling a mission, we may call you directly; The average mission is 250-350 nautical miles one way.
• With few exceptions, flights will be for a patient/passenger and one support person.
• Most missions are of a medical nature but some may be humanitarian. These may include transporting a child with a life-threatening illness to a special camp, someone to see a dying family member, disaster relief, etc.
• Patient/passengers must meet certain mission criteria before they qualify for a flight, they must demonstrate financial distress with limited or no other means or funds for transportation, have a Physician’s Release for the flight. be ambulatory, able to board an aircraft with little or no assistance, be able to sit upright and sign a General Release of Liability and Indemnity before we will consider their request for a Mission.
