Criteria for Passengers

To qualify for a LifeLine Pilots' mission a passenger must meet the following criteria.

· have a medical or humanitarian need far from home.

· have a financial need for assistance.

· be able to fly in a non-pressurized aircraft.

· be able to board the aircraft with minimal assistance.

· be able to sit upright in a standard aircraft seat.

· be willing to sign a liability release form.

· provide own transportation to and from the airport.

A mission coordinator will provide the passenger or contact person with the appropriate paperwork to request a mission.  Mission coordination can be contacted at 1-800-822-7972 or by email at missions@lifelinepilots.org

Requesting a flight

Your mission request begins right now.  If you would like to follow this with a phone call to a Flight Coordinator, please use our toll-free number, 1-800-822-7972.  Our office is open Monday through Friday, 7:30 a.m. to 4:30 p.m. Central Time for general information and requests.
 

Please understand that this is to determine if you fulfill our criteria for a free flight.  If it appears that you qualify, we will email, fax or mail you a General Flight Authorization , Liability Release and Indemnity Form and other informational pages.  Please read these carefully.  We also have a Physician's Release Form to be filled out by your attending Physician and a Financial Verification Form to be completed by an independent third party (not a friend, relative, or neighbor.)  Once all forms have been signed and returned, we can proceed with setting up a flight.  We will do our best to help.

If we are not in the office at the time of your call, please leave your name, location, and phone number with area code on our answering machine and we will return your call.
